Document neurologic symptoms: weakness, sensory loss, atrophy, fasciculations, or abnormal reflexes; specify suspected diagnosis.

Bilateral routine EMG/NCS ordered when unilateral study would adequately answer clinical question

Bilateral testing increases cost; justified only if bilateral pathology suspected or involved extremity unclear.

How to prevent this

Reserve bilateral studies for suspected generalized neuropathy, bilateral compression, or when clinical exam unclear; unilateral usually sufficient initially.

Repeat EMG/NCS within 6 weeks of prior study without acute clinical change or treatment intervention

Occasional

Repeat studies require interval clinical change or post-treatment assessment (post-injection, after conservative therapy trial).

How to prevent this

Space studies 8-12 weeks minimum unless clinical deterioration or acute intervention (injection, surgery) occurred.
