EMG / Nerve Conduction Study — 1-2 Nerves
What is this procedure?
Electrodiagnostic testing that measures electrical activity in muscles and nerve conduction speed. Used to diagnose carpal tunnel, neuropathy, and radiculopathy.
Does this require prior authorization?
Step Therapy / Pre-Requirements
Some plans require failed conservative treatment (splinting, medication) before approving EMG for conditions like carpal tunnel syndrome.
Common Reasons This Gets Denied
Based on insurer policy analysis and claims data patterns. Frequency indicates how often this reason appears.
EMG/NCS ordered without documented peripheral neuropathy symptoms or neurologic deficit
EMG/NCS justified by suspected nerve injury, polyneuropathy, or muscle disease with clinical signs and symptoms.
How to prevent this
Document neurologic symptoms: weakness, sensory loss, atrophy, fasciculations, or abnormal reflexes; specify suspected diagnosis.
Bilateral routine EMG/NCS ordered when unilateral study would adequately answer clinical question
Bilateral testing increases cost; justified only if bilateral pathology suspected or involved extremity unclear.
How to prevent this
Reserve bilateral studies for suspected generalized neuropathy, bilateral compression, or when clinical exam unclear; unilateral usually sufficient initially.
Repeat EMG/NCS within 6 weeks of prior study without acute clinical change or treatment intervention
Repeat studies require interval clinical change or post-treatment assessment (post-injection, after conservative therapy trial).
How to prevent this
Space studies 8-12 weeks minimum unless clinical deterioration or acute intervention (injection, surgery) occurred.
Documentation Checklist
Gather these documents before submitting your authorization request. Click items to check them off.
Documentation of specific neurologic symptoms (weakness, sensory loss, numbness, tingling)
RequiredClinical indication for nerve/muscle testing
Specific nerve(s) or muscles of concern identified based on symptoms/examination
RequiredGuides which extremities and nerves to test
Prior EMG/NCS results with dates if repeat study
Strongly RecommendedEstablishes interval change justifying repeat
Neurology or neurosurgery consultation note supporting indication
Strongly RecommendedSpecialist confirmation of indication
Medical Necessity Tips
What clinical evidence supports approval
- Document symptoms suggesting nerve or muscle disorder (numbness, weakness, tingling)
- Include physical examination findings (Tinel sign, Phalen test for carpal tunnel)
- Note duration and progression of symptoms
- Explain how EMG results will change management
What to Do If Denied
If your emg / nerve conduction study — 1-2 nerves is denied, you have the right to appeal. Most denials are overturned on appeal when proper documentation is provided.
