Medical Necessity Tips

What clinical evidence supports approval

  • Physical exam confirming reducible or incarcerated inguinal hernia
  • Document symptoms: pain, bulging, activity limitation
  • Imaging (ultrasound or CT) may be needed for occult hernias
  • Incarcerated or strangulated hernias require emergent repair without PA

What to Do If Denied

If your inguinal hernia repair (open) is denied, you have the right to appeal. Most denials are overturned on appeal when proper documentation is provided.
